Please note that our Website is not intended for visitors located in the EEA, as My Health Onsite is a U.S.A. entity providing services in the U.S.A. Any personal information collected about EEA visitors through the Website is processed in the United States by us or a party acting on our behalf. When you provide personal information to us through the Website, you consent to the transfer, storage, and processing of your personal information in the United States.
7. CONTROLS FOR DO-NOT-TRACK FEATURES
Most web browsers and some mobile operating systems and mobile applications include a Do-Not-Track (“DNT”) feature or setting you can activate to signal your privacy preference not to have data about your online browsing activities monitored and collected. At this stage, no uniform technology standard for recognizing and implementing DNT signals has been finalized. RECORD SEGREGATION AND INTEGRATED CARE MODEL
Due to the integrated nature of services provided:

Medical, occupational health, wellness and dispensary records are maintained in the medical record system.

Behavioral health therapy records are maintained in a separate confidential system.

Therapy records are not automatically included in general medical, occupational, or dispensing disclosures.

Only clinically necessary summaries may be shared between providers for treatment coordination.
YOUR RIGHTS
When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you.

Reproductive Health Information – Enhanced Federal Privacy Protections (2024 HIPAA Final Rule)
In accordance with updated federal HIPAA regulations, this practice provides additional protections for Protected Health Information (PHI) related to reproductive health care.
Reproductive health information may include, but is not limited to:

Where applicable, this practice will require a valid written attestation confirming that a request for reproductive health information is not for a prohibited purpose before any disclosure is considered. These protections apply to all medical, occupational health, therapy, laboratory, and medication records maintained by this practice, including records stored in separate electronic systems.
Mental Health Records / Psychiatric Information (Florida Law)
This practice provides therapy services exclusively by a licensed psychologist. Mental health and therapy records are protected under Florida Statutes §§394 and 456 and applicable confidentiality laws. Behavioral health records are maintained in a separate confidential electronic health record and are not automatically included in the general medical, occupational health, or dispensary record.
Mental health records will not be disclosed without specific written authorization from the patient, except as permitted or required by law.
Psychotherapy Notes (Highly Restricted - Federal Protection)
Psychotherapy notes are afforded heightened protection under HIPAA (45 C.F.R. §§ 164.501 and 164.508). These notes:

Are maintained separately from the general medical record

Are not included in standard medical record releases
-
Require explicit, separate written authorization for disclosure
Psychotherapy notes will never be released to employers, third parties, or external entities without a legally compliant authorization, except in limited circumstances permitted by federal law.
HIV/AIDS-Related Information (Florida Statute §381.004)
Information related to HIV testing, diagnosis, or treatment is subject to strict confidentiality protections under Florida law and will not be disclosed without specific written authorization, except as permitted or required by law.
Genetic Testing and Genetic Information (Florida Statute §760.40)
Genetic testing results and genetic information are protected under Florida law and require specific written authorization prior to disclosure unless otherwise permitted by law.
Employer-Sponsored and Occupational Health Confidentiality Limitations
For patients receiving employer-sponsored or occupational health services, this practice limits disclosures to the minimum necessary information required for:

This practice operates an in-house dispensary that provides non-controlled medications as part of integrated patient care. Medication records are considered Protected Health Information and are safeguarded under HIPAA and applicable state confidentiality laws.
Medication and treatment information will not be disclosed to employers, third parties, or external entities without proper authorization, except as permitted or required by law.
More Restrictive Laws and Enhanced Confidentiality
Where federal or Florida law provides greater privacy protections than HIPAA, this practice follows the more restrictive legal standard. This includes, but is not limited to:

Genetic testing information
If a law requires additional patient authorization before disclosure, this practice will obtain that authorization prior to releasing any protected information.
